Confidentiality is our policy. As a requirement of the Federal Privacy Act of January 1, 2004 and as part of the Personal Information Protection and Electronic Documents Act (PIPEDA), our office has developed a privacy policy. If you would like a copy, please request one from our information officer, Dr. Townshend. It documents the collection, use and disclosing of personal information in our practice. Everything you discuss with us is kept in strict confidence. Our team respects this confidentiality and uses the information they are given to schedule appointments effectively and relay important information to your doctor.
Your optometric records will not be disclosed without your consent with the following exceptions:
- We must report a failure to meet driving visual standards and suspected child abuse
- Your consent is assumed when referring to specialists and letters to your family doctor
- We will provide your prescription to licensed dispensers if requested without contacting you first
The privacy of our patients is of upmost importance to us. We are committed to protecting the personal information you provide to us.
In order to provide the best service and care possible, Van Ymeren, Townshend, Lawrence and Associates will collect some personal information such as home address, telephone number, OHIP number and personal health history. Collection of this information will assist us in keeping current and relevant information on file.
